Alexandria Area High School
Sports Physicals may be submitted by email, fax or in person. Please scan and email your student's sports physical to the Activities Secretary, or fax it to the high school at the fax number below, or drop it off in person. A current Sport Qualifying Physical must be completed and on file in the Activities Office every 3 years.
